How Do I File An expungement request?

The Air Force - Criminal Justice Information Center (AFCJIC) has created three templates; one for civilians, one for current service members and one for former service members. The instructions are located at the end of each form. These templates can help you to gather all the required information and advise you how to submit your expungement request. Civilians, current service members and former service members can use these templates to coordinate and request an expungement or correction of their records following an Air Force investigation.

Expungement Forms:

Submit your request via one of these two methods:
1. E-mail to:  daf.cjic.expungements@us.af.mil    
2. Mail to: AFOSI Attn: AFCJIC Expungements, 27130 Telegraph Road, Quantico, VA 22134
 
NOTE: DAF Form 235, DAF Form 236, DAF Form 237 and DAF Form 238 must be downloaded first to a computer and then opened in the Adobe Application.

SPECIAL NOTE: Signatures on ALL forms must have a wet signature (pen to paper) or digital signature using CAC Certificates. Regular digital signatures are not accepted. A scan or photograph of a wet signature is fine.

DAF Form 238 - Verification of Identity (This form is REQUIRED to be submitted with one of the templates below)

DAF Form 235 - Current Service Members Template (If you want DNA removal then you MUST select that section on the form. Without a box being checked in that section we are unable to process a DNA removal request.)

DAF Form 237 - Civilians Template (This form is only to be used by requestors who have never been in the military) 

DAF Form 236 - Former Service Members Template (This form is only to be used by requestors who have never been in the military)
